Care Adoption Contract

ADOPTER'S AGREEMENT

I agree that the dog is being adopted for myself and will not be sold, adopted, or given to another party.

I agree to have the dog examined by a licensed veterinarian within 3 weeks of adoption. From the date of this contract, I agree to assume full responsibility for the veterinary care of this dog. CARE takes no responsibility or warranties regarding the health of this dog. I will take the dog’s vet records to the check-up and ask my vet to complete a wellness exam and discuss beginning flea/tick and heartworm prevention. If my veterinarian finds any problems which warrant concern, I have the choice of returning the dog to CARE for medical treatment or assuming financial responsibility myself. I agree to keep my dog current on heartworm prevention for the life of the dog.

I agree the dog(s) will not be allowed outdoors without proper supervision. When taking my dog outdoors, they will be on a secure harness/collar and/or in a secure fenced area, and wear proper ID I agree the dog will reside in my home and be kept as an inside Pet

I agree that this dog is to be a companion animal, not a guard dog.

I agree to care for the dog in a humane manner and be a responsible dog guardian. This includes supplying adequate food, water, shelter, attention, and medical care, yearly physicals, monthly heartworm prevention, and vaccinations current. I will keep the dog free of irritating pests such as ticks and fleas. I will provide the dog with a humane means of exercise. He or she shall not be kept on a chain or tied outside. I will not leave this dog in an electric containment system unsupervised.

I agree & understand there is a two-week trial period starting on the “Adoption Date” for both the “Adoption Agency” “CARE” and the “Adopter” listed on this contract. This trial period includes the following terms: If for any reason the “Adopter “or a CARE representative decides the dog listed in this contract is not a good fit, the Adopter is required to return the dog to CARE and the “ADOPTER will be refunded 100% of their adoption fee within the dates of the trial period. I further understand CARE has the right to reclaim the dog during the (2 week) trial period and the “Adopter” will be refunded 100% of the adoption fee

I understand and agree throughout the life of this pet if a situation arises, I can no longer care for, or provide for the adopted pet listed in this contract under all circumstances, I am required and agree to contact CARE and return the pet to CARE, ONLY. I agree and understand under all circumstances I cannot “give, sell, or trade this pet to any individual or agency. CARE will be happy to take into consideration the adopter in placing the pet in a new home. CARE will facilitate the placement of the pet and require contract approval. I also agree to transport the dog to the location of C.A.R.E

I understand and agree that Chances Angel Rescue makes no guarantees about the dog/cat’s temperament and is not responsible for future damages or injuries caused by the dog.

I give Chances Angel Rescue permission to call my home at any reasonable time to assure that the dog is being properly treated and cared for.

I agree to keep Chances Angel Rescue informed of my current home address and phone number.

I consent to the examination of this dog by CARE at any time and acknowledge that CARE has the right to reclaim this dog at any time for failure to comply with all terms of this contract or for misrepresentation of facts I made on the adoption application or in this contract. I agree to pay CARE for all expenses, including court costs and reasonable attorney fees, if legal action is necessary to enforce the terms and provisions of this contract.

The officers and volunteers of CARE remain interested in the welfare and wellbeing of your dog and his or her new family. Please contact your adoption coordinator if you have any questions or concerns. Updates of your dog and photographs are always greatly appreciated. Please consider us part of your extended family.
